    A 2010 explosion that killed two workers at a former Horsehead Corp. zinc oxide plant in western Pennsylvania was caused by recurring problems with a sump system that the company failed to address, the U.S. Chemical Safety Board reported. "Human factors played a dominant role" in the explosion at the plant near Monaca, about 30 miles northwest of Pittsburgh, according to the report by William H. Hunter, a British metallurgist hired to study the blast.

    A mother and daughter from Beaver County have been charged with selling prescription anti-anxiety medicine to a Central Valley football player the day before he overdosed and died, police said. Danielle Hrelec, 41, and Brandi Spohn, 20, who are listed in their criminal complaints as living in the same Center Township house, were each charged by Monaca police Feb. 20 with a felony count of possession with intent to deliver a controlled substance.
