Actually, if Tiny Tim suffered from vitamin D deficiency, then his entire family should have shown symptoms too. Tim's mother probably breast fed him for some time after birth, and she had enough vitamin D through breast milk to nourish him. I've always thought that Tim had a genetic predispositioned medical condition. The Crachit diet would have had enough milk, cheese, and cream to supply enough vitamin D to all, hence no symptoms in the father, mother, and older siblings. It could be that Tim developed a renal condition due to genetic variation in his physiology creating susceptibility that other family members did not have in their genetic makeup.

He walked with a crutch. This suggests a bad leg.

Either that, or his father, Bob Cratchit could only afford one shoe.
