The two ways Pam could've given Hef a heart attack would be:(1)Produce a document where he promised to marry her, or,(2)Tell him she's having his baby.

3. He was so shocked at seeing all her wrinkles (since there was no special lighting to hide all the ravages of aging) that he almost had a heart attack. After all, he is used to being surrounded by YOUNG flesh. LOL
