Joining the OMMP, the Oregon Medical Marijuana Program.
***

You must be an Oregon resident to be a registered patient in the Oregon Medical Marijuana Program (OMMP).
You must have a qualifying debilitating medical condition as listed on the Attending Physician's Statement.
Debilitating Medical Condition: your physician Checks the appropriate boxes on the ATTENDING PHYSICIAN’S STATEMENT (see form )
1. Malignant neoplasm (Cancer)
2. Glaucoma
3. Positive status for Human Immunodeficiency Virus (HIV) or Acquired Immune Deficiency Syndrome (AIDS)
4. Agitation due to Alzheimer’s Disease
5. A medical condition or treatment for a medical condition that produces for a specific patient one or more of the following:(check all that apply)

a. Cachexia
b. Severe pain
c. Severe nausea
d. Seizures, including but not limited to seizures caused by epilepsy
e. Persistent muscle spasms, including but not limited to spasms caused by multiple sclerosis

Your physician must be a Medical Doctor (MD) or Doctor of Osteopathy (DO) licensed to practice medicine in Oregon. You must have an established patient/physician relationship with your "attending physician." Naturopaths, chiropractors, and nurse practitioners cannot sign the documentation.
The OMMP cannot refer you to a physician. The OMMP does not have a physician referral list.
You must list a grow site address on your application. You, or your designated primary caregiver, may grow your own medication. There is no place in the State of Oregon to legally purchase medical marijuana.
The OMMP cannot find a designated primary caregiver for you. The OMMP does not keep a referral list of persons who want to be caregivers for patients.(You are not required to list a caregiver, unless you are less than 18 years old.) Your caregiver cannot be your physician.
The OMMP cannot supply you with seeds or starter plants, or give you advice on how to grow medical marijuana.
The application fee cannot be waived. Partial payments cannot be accepted.****** If you have one of these qualifying conditions contact Voter Power or Compassion Center for an appointment.

Hello, My name is Marc I am a 50 year old male who is currently prescribed morphine sulfate (MS Contin) on a daily use (30mg to 100mg) from the University of Wisconsin now for over 3 years which I take daily for on going pain (2-hip replacements, shattered femur, nerve damage and back problems). Before that it was other type of pain meds for my injuries. I have tried marijuana at times and notice that it has made me more comfortable and gave me an appetite. I have lost 55lbs over the last few years while on the morphine for I do not have an appetite. I have currently on SSD for the past 11 years and I do work part time. My question is how or can I obtain a Medical card for marijuana while I live in Wisconsin? I did live in Washington State for 43 years before coming to Wisconsin 7 years ago. Any help or directions are greatly appreciated. Sincerely Marc

Here are the current laws for Marijuana Possession in Wisconsin.-- http://norml.org/index.cfm... Apparently your state is not a Compassionate State and has no medical Marijuana Defense. Even though you definately qualify under Oregon laws you are subject to prosectuion in your state even for Medical reasons. Here is a good place to start helping turn things around there. Join NORML or http://www.drugsense.org/dpfwi/

I'm a 33 year old man who is a long time resident of Oregon. I've been suffering from severe pain due to several car accidents and a hit and run in the past 10 years. I've had extremely painful migraine headaches since I was 23, the kind that render me useless for hours. I recently had 5 inch needles inserted into my back and neck all the way to my vertebrae to inject a mild irritant while awake in a procedure called "Prolotherapy". I was given 10 injections in one session. This didn't seem to help. I also suffer from a degenerative disorder in the vertebrae in my neck, which is slightly crooked. On top of that is your basic neck and back pain from several whiplashes in MVA's. I also experience joint pain in my right shoulder daily from a hit and run. I've been prescribed several different kinds of opiates. Now I'm taking Norco, Tramadol (Ultram), and Ibuprofen. I'd like to get away from the opiates and try something less damaging to my body. Do I sound like I would qualify? Thanks for your time.

http://www.voterpower.org/ Jean Claude,Voter Power can and will help you. You especially with the conditions you have listed(Chronic Pain) you would qualify for an Oregon Medical Marijuana Card.Be sure and take all documentation/history you have of your qualifying conditions with you to the Voter Power meeting. They have clinics in Portland regular and also travel to smaller cities for clinics.

I HAVE JUST RECENTLY BEEN PRESRIBED, PROZAC AND ANOTHER DRUG I CANNOT SPELL FOR ANEIXTY AND DEPRESSION, I AM TAKING THESE MEDICINES EVERYDAY, AND AM TERRIFIED TO BECOME HOOKED ON THESE MEDS. AS WELL AS ADDING A CHEMICAL TO MY BODY THAT ISN'T NEEDED. I HAVE FOUND THAT IT IS THE EASIEST WAY FOR A DR. TO SHUT THE PATIENT UP! "HERE TAKE A COUPLE OF THESE AND YOU WILL BE FINE, THEY A BETTER THEN THERAPY" QUOTE FROM MY DR..I HAVE READ HORROR STORIES ABOUT COME OFF FROM PROZAC. I FIND THAT MARIJUANA IS THE BEST WAY TO ATTACK ANEIXTY, AND DEPRESSION. AS WELL AS NOT RELYING ON CHEMICALS TO GET ME THROUGH THE DAY. I WOULD BE INTERESTED IN GETTING A MEDICAL CARD FOR THIS BUT FEAR REJECTION, DUE TO IT NOT BEING ONE OF THE "9 CONDITIONS" NEEDED, ANY ADVISE ON FINDING OUT HOW TO TAKE THESE ACTIONS FURTHER?

The Horror Stories/Withdrawels from PROZAC are true,I can tell you from experience.I saw horrible things, nightmares, halluciation s and the anxiety/depression came back hundred fold. It was as if all the problems I had masked with PROZAC for a couple of years had all come back at me at one time.I have never found anything to help alleviate my depression as much as Cannabis. Unfortunatly this is not a qualifying condition for an Oregon Medical Marijuana Card. Please contact the DHS any how and tell them of your situaion and that you would like to see them include depression/anxiety as a qualifying condition.
