You must have read it wrong.
And to answer "concerned one," the "root cause" is cited right there. The DRIVER, who OPERATED the truck that hit them, was CITED for failure to use due care. The ROOT CAUSE of this accident was identified by the responding police officer.

I think "concerned one" is the type that if a felon broke into my house and threatened me an my family, and I pulled a firearm and killed that felon, the incident would be blamed on the firearm or me, but somehow NOT the criminal who is actually performing a CRIMINAL act.

This curse will endure until the dishonorable deed is corrected and the Stadium is once again properly named "Joe Robbie Stadium" - once and for all.
