Pat - your doctor sounds like he knows what he is doing. He will most likely keep you on the Lovenox after your surgery, until you have restarted your coumadin, and your INR is in range. The lovenox is to give you protection while you arent on the coumadin.

My mother was going to be released from hospital on 07-3-09 and a prescription was called in for Lovenox 10MG twice daily. She was not released until 07-10-09 and a prescription was called in for coumadin 12MG a day. Well she had a bleeder in her brain on 07-14-09. She passed and the medical examiner said those medications should have never been mixed. So I would strongly suggest no one mix the two, but I am not a Doctor. I know a Dr. that killed my mother.

Those are really high doses individually, so I imagine that combining them would be bad. I'm really sorry to hear about your mom.

To respond to the original question, when I was in the hospital, I was on Lovenox. As I was getting close to being released, I was on lovenox and coumadin until my levels were stable. Once my INR was within range, I was taken off the lovenox. Though, if my INR was ever too low, my hematologist would give me lovenox until it was back in range. I never had any complications. I actually preferred Lovenox as you didnt has as many restrictions as you do when on Coumadin

Mike - Im sorry to hear about your mother. The medical examiner is wrong however - Lovenox is not normally given continuously with coumadin - it is used as a coverage until your INR is in range with coumadin.

my father was stable on coumadin for 20 yrs. in preperation for a kidney biopsey he was perscribed Lovenox .8MG twice daily--despite decreased kidney function. he had a brain bleed 10 days after his first injection and has been in the hospital for 5 months and is still critical. make sure doctors conduct filtration tests to measure Lovenox levels in the blood!

In response to Laurie, I was on Lovenox for years, mainly becuase of pregnancies after a PE, but for about a year because they could not get my coumadin levels normal from being on high doses of prednisone for asthma. Lovenox was the only way to go at that time.
I just had another round of dvt/pe and now I have a filter and will be in coumadin forever. My problem is I'm getting married next month and everyone knows it's coumadin law not to get pregnant on coumadin. My concern is switching from coumadin to lovenox. How did it work during your pregnancy. Really scared to get pregnant cause I almost died twice and I'm only 27!

Quite safe to take both at the same time... they work in two different ways and you quit Lovenox when your PT/INR is therapeutic with Coumadin.
I don't quite understand his logic in that Lovenox is "stronger" though. Therapeutic range is normally what is sought with Coumadin.

I am on a new thinner Pradaxa for my DVT. I stopped taking warfarin because of chest pains, numbness in left arm and shortness of breath. I was hhospitalized 5 X and visited 11 ERs due to the side effects of warfarin which is why they took me off.
Because the doctors discharged me w/o bridging the transition, my DVT came back. Only been on new drug for 1.5 days. My question is can I take Lovenox and Pradaxa together? Since the drug is only 1 month old, there is no study on the usage of both, neither is there an antidote if my levels goes too high. Help,

My wife has been on Lovinox (enoxaparin) for six months as she went thru chemotherapy (Taxol and Avastin). She was put on it due to emboli seen in her lungs during her pre-chemo PET scan. She is now in a clinical trial for Avastin (only), and the doctor wants her to stay on Lovinox for the forseeable future. Does anyone know why she can't move to coumadin to save her the pain and hassle of self-injections twice a day, as well as the exhorbatant costs?

I have been on coumadin for ten years and every time I have invasive surgery I have to stop the coumadin and take 120mg lovenox at 12 hour intervals five days prior to surgery. This dosage is determined by your weight. 24 hours after surgery you start the lovenox and the usual dose of coumadin. Then after five days you go to the lab to get your pt/inr. My problem was excessive bleeding after resuming the lovenox and coumadin after surgery. Went to my Hemotologist's office for some advice, and sat in the lobby for twenty minutes, with no advise. I went to my primary doctor to inform her what was happing. I feel that I am on my own. I have cut the lovenox dosage in half and continue to take the usual 6mg of coumadin per day. Yesterday I checked my pt/inr and it was 1.5. I need to be between 3.0 and 4.0. So, the excessive bleeding has stopped and in several days I will go again for a pt/inr. I am really disappointed with my Hemotologist because I feel that I have been left on my own to make decisions that I should not be making. I will be getting a new Hemotologist and will be better informed to ask questions beforehand, with regard to any complications that may arise after surgery. I am trying to stay on top of this treatment, but you need the cooperation of the doctors involved. I am pro active and will be seeking the advice of another Hemotologist Monday.

Hi
when a clot was diagnosed in my leg below the knee. My Dr. put me on Lovenox asap, while the coumaden builds up, taking approx 3 days.
For my leg clot I was put on Lovenox I took
1 coumaden a day strength 9, for 3 days. Then switched to 1 pill a day strength 5 continuous. It took 5 days to get my INR range between 2-3. I used Lovenox and coumaden together for the 5 days and had no problems.

Your case may have different symptoms. You may need emergency help if the clot is in the upper leg or lung. In the lung, heart and brain can be fatal. YOU will need to see your DR asap what ever the case, clots are serious and nothing to mess with, they need attention immediately.

Concerning surgeries while on coumaden before surgery the DR gave me blood plasma and reduced my INR to normal in about 4 hours. After surgery I had to go on Lovenox and build the coumaden back up again.
Your doctor is the one who needs to give you info, It is a serious problem. You do not want to get your blood to thin. You must have regular blood checks to keep it in the correct range.
God Bless
