The pilot is the reason the plane went down. He failed to cycle the boots, standard practice in icing conditions. At no less than four different times they should have been cycled, they were never cycled at all through out the flight. Boots are air bladders on the leading edge of the control surfaces that expand to remove ice. They were not used, failure by the flight crew.

The lack of actions from the cockpit AND the FAA's knee jerk issued mandate !! In 1999, the FAA issued an Airworthiness Directive (AD 99-19-18) that mandated revisions to AFM to advise flight crews to activate airframe pneumatic de-icing boots at the first sign of ice accumulation. This system is from 1933 by design. The FAA fails to understand the word "BRIDGING". You need to operate the boots as they were designed. The ICE accumulates - turn the boots on to breaks the ICE off - turn the boots OFF Repeat when ICE builds NOT turn ON an forget. |
