You do as they do in some Asian nations, you give a heroin addict a one shot chance at recovery. That's it, no more, nada.
No substitutes , cold turkey in a controlled environment.

If they relapse , they are taken out in the boonies and shot. End of story.

I am a recovering alcoholic and no one gave me a substitute for my booze. I know trhe program hates my attitude, but everyone knows what heroin does and at least with alcohol, you can almost state that it is an accepted drug albeit no better than heroin.

I'm 14 years sober and I have NO sympathy for a heroin addict.
