One of the less-reported stories involves an accident the occurred during the outage, where a worker fell into the San Onofre Reactor 2 reactor pool on January 27th. David Lochbaum, who worked in nuclear plants for 17 years before joining the nonprofit Union of Concerned Scientists, said he believes small particles of uranium fuel —- sometimes called “fuel fleas” or “hot particles”—- could have been in the San Onofre Reactor 2 reactor pool when the worker fell in.
After looking at a set of reactor operation statistics published on the NRC’s website, Lochbaum said the radiation levels in the plant’s cooling system doubled from January to February 2011 and continued to climb through the end of the year. He shared his findings that increasing levels of radiation in the water that cools the reactor probably indicates that the metal tubes — called cladding — that contain its fuel pellets are cracking. Those cracks, he said, could have released particles into the reactor pool when uranium fuel was removed from the reactor core during refueling, well before the worker’s fall. “The chart shows that San Onofre Unit 2 had operated for several months with degrading fuel cladding,” Lochbaum said.“The opening in the fuel cladding that allowed radioactivity to escape into the water at ever-increasing rates could have also been a pathway for fuel fleas —- small particles of fuel pellets —- to escape.” |

Cladding damage can come from: 1) poor chemistry coming to IGSSC or intergranular stress corrosion cracking 2)overpowering a pin 3)debris circulating in coolant and sandblasting the cladding away... |

WASHINGTON, Feb. 28 (Kyodo)— Cracks and damage were found in a Japan-made steam generator for a nuclear reactor in Southern California in 2009 before its delivery to the plant, U.S. Nuclear Regulatory Commission documents showed Tuesday. The steam generator, produced by Mitsubishi Heavy Industries Ltd., was installed at the unit 3 reactor at the San Onofre nuclear power plant in California, which suffered a minor radiation leak in late January. It remains unclear whether the cracks and the radiation leak were related. At the nuclear plant, operated by Southern California Edison, abnormal attrition was also found earlier this month in the tubes of another Mitsubishi-made steam generator installed at the unit 2 reactor. ADVERTISEMENT Mitsubishi Heavy and Southern California Edison both declined to comment, saying they are under investigation. Edwin Ryman, a nuclear expert at the Union of Concerned Scientists, said the problems could be attributed to either defects in the part or inappropriate operation. A steam generator, composed of fine tubes, is a critical part of a pressurized water reactor. According to the NRC documents, cracks and damage were found in the steam generator of the unit 3 reactor during checks by Mitsubishi Heavy in March 2009. A follow-up probe found heat treatment prior to welding caused the problem and the NRC informed the utility and others concerned. |

Edison utility said Friday four steam tubes, or metal pipes, that carry radioactive water failed pressure tests. Three other tubes ruptured during testing earlier in the week, prompting the Nuclear Regulatory Commission to send a team to investigate. SoCal Edison said it was taking the ruptures seriously, and was looking for alternative power sources to serve customers. The utility said it would continue testing more than 120 similar tubes before making a decision as to how or when it might restart the plant. Operators of the plant, located near San Clemente, Calif., shutdown Unit 3 on Jan. 31, after a tube leak in one steam generator was identified. Resident inspectors responded to the control room to monitor site activities during the incident. The unit has remained safely shutdown and NRC has had a steam generator expert and additional inspectors at the site inspecting the licensee’s activities and tests of the steam generator tubes. |

Things are still in the very early stages of inspection, hell the unit at this point hasn’t even fully cooled. This will not be solved in days, or even weeks.
