This is a sad incident. There often many small safety percautions that can be taken to avoid such accidents, I wouldnt say that it was Newmonts fault that he indeed died. All employees on company property must remain safe and alert at all times.

I think that is what happened... Everyone gets so use to hearing back up alarms and the horn honks so many times a day they become complacent. Complancy leads to death... and thats terrible!
