By DUNCAN MANSFIELD The Associated Press KNOXVILLE, Tenn. - A report says the Tennessee Valley Authority's response to a massive coal ash disaster last year was hampered by emergency communications problems, inaccurate statements to the media and delayed reparations to victims.
